Licensed Practical Nurse (LPN):
LPN's work in clinical settings to provide client care. A LPN may conduct and document initial and on-going focused nursing assessments of the health status of clients. They focus primarily on triage to assess clients' health status for immediate care, clinical appointment, education for care at home, or referrals to resources.
Registered Nurse Care Coordinator (RNCC) with Community Health Centers:
RNCC's are part of a multidisciplinary care team. They provide care coordination and case management, health education, nursing assessment, patient triage, and medication counseling. Nursing care is provided to clients of all age ranges.
Nurse Positions may be filled at the following levels depending on candidates' qualifications.
- LPN - Licensed in the State of Oregon with 1 year of exp.
- Nurse 1 - RN
- Nurse 2 - RN with 4 years exp. -or- BSN with 1-3 year exp.
- Nurse 3 - RN with 14 years exp. -or- BSN with 8 years exp. -or- MSN with 8 years exp.
Equivalent combination of education and specialized experience may be accepted. Please be sure to indicate any experience in pediatrics, care coordination, home visiting, mental health and language skills.
- LPN (HCP 1): $24.43 - $36.35/Hour $50,824 - $76,230/Annual
- Nurse 1 (HCP 2): $25.65 - $38.49/Hour $53,358 - $80.049/Annual
- Nurse 2 (HCP 3): $26.94 - $40.41/Hour $56,031 - $84,046/Annual
- Nurse 3 (HPC 5): $30.22 - $45.33/Hour $62,851 - $94,276/Annual
